Crew Requests
Please note: Requests must be entered 2 weeks in advance and are not guaranteed. Any individual that excessively requests off weekends will be removed from the schedule indefinitly. Any availability changes that are needed must be verified by the GM only. You will remain on the schedule per your original availability until this happens, and will be held accountable for your schedule if any days are missed. Therefore, it is imperative that you communicate directy to the GM via Next app or using the notes on this request sheet. Due to current staffing needs, there are only 6 availble slots for weekday request and 3 available slots for weekend requests. If you have a special need such as a vacation or doctors appointment, but didn't request in time, please leave a note next to that particualr week so the GM can review your requests!
